Social Determinants of Health Conference
8:00 am - 4:00 pm
Desert Willow Conference Center
The low-hanging fruit has been picked and it’s time to focus on the more complex factors leading to readmission: Social Determinants of Health. Arizona’s hospital readmission rate has decreased to 15.7 percent (in comparison to an 18.4 national average), thanks to the efforts of health care professionals statewide. What comes next is work that lies at the intersection of health care and the social determinants of health. Join Health Services Advisory Group, Vitalyst Health Foundation and the Federal Reserve of San Francisco on March 27 from 8:00am-4:00pm as we examine strategies that improve care coordination, decrease readmissions and improve health.
Anyone with a stake in increased care quality is encouraged to attend: hospital and clinic leaders; directors of case management, pharmacy and quality; home health agency owners and clinical directors; community health providers such as pharmacies and care homes; and community-based organizations and stakeholders.
Learn about innovative strategies addressing housing, transportation and food security from keynote speakers and panelists, connect with the inspiring story of how a single shoebox inspired an entire movement, and leave with a framework for exploration and collaboration going forward. Register today, and we’ll see you at Desert Willow Conference Center on March 27.
